The old guy, given he's standing farther back from the camera, is almost level from head top to head top with the furry guy. From what I can tell, there appears to be no lift where the old guy is standing, and he's even HUNCHED OVER so either the furry guy is really short or shopped in without fully considering perspective.
On top of that, him and grandma are relatively close to the flash (the furry guy is just a tad farther back) yet grandma's skin is way more luminous compared to the furry guy.
But I'm looking for reasons why this is shopped in, since it's really the situation that makes no sense. Either it was shopped or set up.

Don't know what you mean about no lift. The hunching can be attributed to bad posture or looking down. He's also more in the background so that may be why he appears really short.
Light source seems to be coming from the left. Furry's left side is fairly bright and it's shady where it should be. Grandma would be getting hit directly by the light as she is.
